Registered and active parishioners of SMOS parish.

One completed parish registration form per family.

One Consent Form A for each student registered.

Copy of each student’s baptism certificate (provided first year of enrollment only; will be kept on file.)

 Registration Fee for 1 child is  $170.

 Registration Fee for 2 children is  $110 (per child.)

Registration Fee for 3 or more children is $96.00 (per child.)

Sacrament formation is a separate program from PREP. Families with eligible children will be sent information regarding sacrament formation.
